Basis for Recommendation

The RAIU review of Signals Passed At Danger (SPADs) occurring during degraded train operations, found that the actions of some third parties were contributory to the incidents occurring in most SPAD events. The RAIU also found that there were several instances of poor communications between operational staff during degraded train operations, and as a result the RAIU make the following safety recommendations:

Recommendation

Iarnród Éireann (Railway Undertaking & Infrastructure Manager) should carry out a review of the interfaces between different operational staff (i.e. drivers, Level Crossing Control Operatives, signalmen and Emergency Operators) so that all operational staff can adequately manage train operations during degraded situations. Part of this review should focus on the safety critical communications between operational staff.
