Basis for Recommendation
The FFCCTV on EMU 8627 was not operating as a fault arose on the system and the scheduled maintenance for the FFCCTV was not implemented until the complete 8520 EMU fleet had received its installation i.e. maintenance tasks were only commenced when the fleet was complete, rather than commence with individual units. As a result the RAIU make the following safety recommendation:
