Investigation Report

An Investigation Report is a formal document that presents the findings of an investigation into a railway accident or incident, including the circumstances and sequence of events, relevant evidence and analysis, the identified causal, contributing and safety factors, and, where appropriate, safety recommendations arising from the investigation.

Urgent Safety Advice Notice (USAN)

A USAN is a communication issued by the RAIU where immediate action is considered necessary to address a significant safety risk identified during the course of an investigation or preliminary examination. A USAN is intended to bring the safety risk promptly to the attention of relevant railway organisations and other stakeholders so that appropriate interim measures can be considered and implemented without waiting for completion of the investigation. After the issuance of a USAN a further Investigation Report may not be warranted.

Safety Advice Notice (SAN)

A SAN is a communication issued by the RAIU to bring identified safety concerns or safety learning arising from an accident, incident or other investigation activity to the attention of relevant railway organisations and stakeholders, with a view to encouraging appropriate action to reduce the risk of recurrence. A SAN may be issued where action is considered necessary or beneficial before completion of the investigation, or in some instances is a stand alone report i.e. after the issuance of a SAN, a further Investigation Report may not be warranted.

Safety Bulletin

A Safety Bulletin is a concise publication issued by the RAIU to communicate safety learning(s) arising from a railway accident or incident. It is intended for events where a full RAIU investigation is not warranted, but where the circumstances of the event provide relevant safety learning(s) for the railway industry. The Safety Bulletin presents the relevant facts, the circumstances of the event and, where established, its causes and contributing factors. A Safety Bulletin does not contain new safety recommendations.
