The in-cab driver indications for the passenger door indicate that the doors are closed, when in fact, they remain open. The safety consequence as a result of this equipment failure could lead to a Dart train being driven while the doors are open; with a risk to passenger safety.
Iarnród Éireann should put in place a system to manage the risks associated with the wrong side failure of the door interlocking equipment on the DART trains.
