The system design of the Mainline CTC Workstation which required two operators was identified as a factor in the incident. This resulted in the task of implementing signal protection being carried out jointly by the two signalmen, requiring a high level of co-operation and co-ordination and introducing the potential for error.
IÉ-IM should conduct a risk assessment of double manned signalling workstations to determine if any additional risks from having two persons operating the one workstation, with a view to identifying additional control measures to mitigate any identified risks.
